What is The Athlete's Forge — Hawai'i?
We are a sports physical therapy and performance lab in Honolulu. We combine evidence-based physical therapy with gold-standard metabolic and 3D biomechanical analysis to engineer recovery and performance — not by guessing, but by measuring. We are the only center in Hawaiʻi offering both INSCYD metabolic testing and MOTIONMETRIX motion analysis under one roof.

What are your hours?
Monday through Friday, 8 AM to 6 PM. Saturday, 8 AM to 12 PM. Closed Sunday.

Insurance, Cost & Booking
What you need to know before scheduling.
Do you accept insurance?
Yes — we accept most major insurance plans including HMSA, HMAA, and UHA. We recommend confirming coverage with your insurer before your first visit, as plans vary.
Do I need a physician referral?
Hawaiʻi allows direct access to physical therapy, so a referral is not required to be seen. However, most insurance companies require a physician referral in order to reimburse PT services. If you plan to use insurance, contact your provider to confirm whether a referral is needed.
How much does a visit cost?
Costs vary by service. Most visits fall in the $180–$250 range when paid out-of-pocket. With insurance, your responsibility depends on your plan's deductible, copay, and coinsurance.
How quickly can I be seen?
Initial visits are typically scheduled within 7 days of referral, often sooner.
How long is a typical visit?
Approximately one hour.
How do I book an appointment?

What is your cancellation policy?
We ask for at least 24 hours' notice for cancellations or rescheduling so we can offer the time to other patients on our waitlist.

Do you only work with elite athletes?
No. We treat patients of all activity levels — from people recovering from surgery to youth athletes, weekend warriors, and elite competitors. What's consistent across every patient is our approach: an athletic model individualized to your physiology, your goals, and your daily life.
What conditions do you treat?
Common cases include post-surgical rehabilitation (ACL reconstruction, rotator cuff repair, hip arthroscopy, total joint replacement, lumbar discectomy), tendinopathies, stress fractures, ligament sprains, muscle strains, overuse injuries, cervical and lumbar pathology, shoulder impingement, hip and knee osteoarthritis, return-to-sport programs, and concussion recovery. I have a chronic injury that hasn't gotten better elsewhere. Can you help?
Often, yes. Our approach focuses on the root cause of injury rather than just the symptoms — using manual therapy, progressive loading, and biomechanical analysis to find what previous treatment may have missed. The metabolic and 3D motion data we collect is the kind most clinics don't have access to.
Do you treat children and youth athletes?
Yes. Youth athletes entering competitive seasons are a core part of our injury-prevention (prehab) program, where we screen for asymmetries, mobility restrictions, and strength imbalances before they become problems.

What services do you offer?

What's the difference between Injury Prevention (Prehab) and Sports Injury Rehabilitation?
Prehab is for athletes who aren't currently injured — we screen for asymmetries, mobility restrictions, and strength imbalances before they cause problems. Rehab is for athletes currently sidelined by acute or chronic pain (runner's knee, shoulder impingement, tendonitis, etc.).
What does post-surgical recovery look like at The Forge?
We work directly with your surgeon to follow specific protocols while keeping the rest of your body conditioned. We manage surgical sites and swelling, restore full range of motion, and minimize muscle atrophy during restricted phases — from ACL reconstruction to rotator cuff repair.
Do you offer telehealth visits?
Yes. Telehealth & Remote Consultation is available for appropriate cases — useful for athletes traveling, recovering between in-person visits, or living outside Honolulu.
Do you offer in-home services?
Yes. Our Concierge In-Home Training brings rehab and performance work to you, useful for post-surgical patients with mobility limits or athletes who prefer privacy.

What is INSCYD metabolic testing?
INSCYD is gold-standard metabolic profiling that maps your physiology in detail — your aerobic and anaerobic capacities, fat and carbohydrate utilization, recovery characteristics, and sport-specific limiters. The output is precise training zones, individualized pace strategies, fueling recommendations, and race pace predictions. We are the only clinic in Hawaiʻi offering INSCYD. What is MOTIONMETRIX 3D running analysis?
MOTIONMETRIX is a markerless 3D motion-capture system specifically for running gait. It measures joint angles, ground contact, asymmetries, and running economy in real time — far beyond what 2D video can show. We use it to identify injury risk factors and improve running mechanics. What other technology do you use?
Stryd (running power), Favero Assioma (cycling power), EO SwimBetter (swim-stroke analysis), Paddlemate (paddle-sports power and stroke analysis for outrigger, kayak, canoe, and SUP), and hdrop (continuous hydration monitoring for land-based endurance). Who is performance testing for?
Competitive athletes (runners, triathletes, cyclists, paddlers, swimmers) looking to set Personal Records, qualify for major events, or break through plateaus. It's also valuable for athletes in rehab to objectively confirm readiness to return to sport.
Can I do testing without doing physical therapy?
Yes. Performance testing is a stand-alone offering. Many of our testing clients are healthy athletes simply optimizing training.
Bike Fit & Aerodynamics
Position, power, and free speed.
What's a 3D Bike Fit?
We use 3D motion capture to analyze your biomechanics on the bike in real time — not a static fit. We adjust your position to reduce injury risk, eliminate discomfort (saddle sores, numbness, joint pain), and maximize power output. The result is a data-driven, repeatable position.
What's a Full Aerodynamic Assessment?
For competitive cyclists and triathletes where every second counts. We use a validated virtual wind tunnel and computational fluid dynamics developed by the former head of aerodynamic design at Specialized — the same system used by top record-setting cyclists and triathletes worldwide. We test and optimize your on-bike position and equipment choices to reduce aerodynamic drag (CdA), so you gain speed without increasing power output.

How do physicians refer a patient?

What status reports do you send back?
Comprehensive progress reports at clinical milestones, including measured outcomes, treatment notes, and timelines. We coordinate care directly with referring providers and surgeons.
What testing batteries do you offer for return-to-sport clearance?
Objective testing including functional, isokinetic, and biomechanical assessments — documented at each milestone — to inform clearance decisions rather than rely on subjective recovery.

What to expect when you arrive.
What should I expect on my first visit?
A thorough evaluation: history, physical exam, and (depending on the service) initial measurements or testing. The first visit is approximately one hour. We'll discuss findings, propose a plan, and answer any questions before you leave.
What should I bring to my first visit?
Comfortable clothing you can move in, athletic shoes if relevant to your visit, your insurance card if using insurance, and any imaging or surgical reports related to your case. If you've been seen elsewhere for the issue, bringing previous notes is helpful.
